We feel that there is a demand in the black community to fulfill a badly underserved industry, the end-of-life care services. That is why we decided to open up Touching Hearts Hospice.
A black owned hospice practice, Touching Hearts Hospice aims to reach out to more minority patients needing end-of-life services.
Touching Hearts Hospice is a member of all of the following:
A lot of the time, patients and families request to receive care in their own home. In almost all hospice care cases, services are fulfilled in the patients home or at a community based facility such as an assisted living facility or a nursing home.
When a patient requires care outside of the needs of a in-home care, Touching Hearts Hospice contracts inpatient care with hospitals and top tier nursing facilities. Inpatient care provides short-term monitoring and treatment of pain and other sysmptoms.
Being a caregiver is a tough, energy expending, stress inducing full time job. Not easy having to take care of a loved one during this time. For those who need a break every now and then, we offer Respite Care. In all that you do, you deserve a break.
At Touching Hearts Hospice, we believe that every patient has different needs. This being said, every single patient has a customized plan created to your loved one's needs.

Touching Hearts Hospice can take the task of coordinating various professionals to look after patients at home. Services may include Nurses, Aides, Consulting Physicians, Social Workers, A minister or other support professionals.
Touching Hearts Hospice understands that the process of loved ones having end of life care, requires the emotional and social support beyond physical symptoms. We work with Counselors and mental health professionals to help loved ones deal with those who are dying.
Our team includes a social worker staff that addresses emotional and psychological matters that come with loved ones who are dying.
